FUNDACION PERUANA DE CANCER

Peruvian Cancer Foundation is a non-profit private organization recognized as a juridical person.
It was constituted on November 22, 1960 before the Lima Notary Public Rafael Chepote C.
It was registered on December 30, 1960 in the Book of Foundations, Juridical Persons Registry, Registry Office of Lima (Public Registries), Book 01, page 180, entry 01; currently Certificate Nº 11007934.
Throughout its institutional life time, Peruvian Cancer Foundation has made two amendments to its Social Statute:

On September 22, 1994, it was registered in Book 01, page 254, entry 07 of Registry Office of Lima (Public Registries).

On February 13, 2001, it was registered in Certificate Nº 1107934, entry C 00007, page 09, Registry Office of Lima (Public Registries); which are our current bylaws.
Our Institution has three governing bodies:
- The Board of Patrons.
- The Board of Directors.
- The General Management.
Its current Chairman is Eduardo Payet M. doctor in human medicine, whose position is registered in entry C 00016, Certificate Nº 11007934, page 18, Registry Office of Lima. Deputy Chairman is Mr. Felipe Barclay P. whose position is registered in entry C 00014 Certificate Nº 11007934, page 16, Registry Office of Lima.
The General Management is on the responsibility of Mr. José Alberto Del Corral P., whose appointment is registered in entry C 00017 Entry Nº 11007934, page 19, Registry Office of Lima.
Likewise, Peruvian Cancer Foundation is registered:

Private Institutions of Assistance and Help of the Ministry
of Health, authorized by Ministerial Resolution Nº 0010/74-SA/ASO.

In the National Administrative Registry
of Foundations, under Nº 13, of Supervisory Board of Foundations
(CONSUF), which is an agency dependent on the Ministry
of de Justice.

In the Registry of Entities Permanently
Exempted from Income Tax, before the National Superintendancy
of Tax Administration (SUNAT), authorized by Intendancy
Resolution Nº 049-4-00181/ SUNAT dated July 17, 2003.

In the Registry of Entities Authorized
for the Collection of Donations, before SUNAT; authorized
by Ministerial Resolution Nº 209-2003-EF/15 and renewed
by Ministerial Resolution Nº 357-2006-EF/15, dated July
03, 2006 of the Ministry of Economy and Finance.
