If you are visiting our web page from overseas, the documentation below is what you need in order to make your donation :

Donations from abroad:

You need the following documents:

1. 1. Donation letter
2. Waybill
3. Technical characteristics of the commodities

1. The donation letter, certified by the the Peruvian Consulate in the country of origin of the merchandise items, must include the following:

• Identification of the donating individual/ company
• Place of origin
• Quantity
• Description of the items being donated.
• Approximate weight of those goods.
• Estimated monetary value of the commodities being donated.

2. Copy of the Documents of Goods Clearance:

• Invoice
• List of Contents (In Spanish)
• Bill of lading.

3. Specifications required to obtain Technical Reports corresponding to all donations coming from abroad:

a) Medicines:

• List of specific medicines, supplies, or drugs, in Spanish language, including the International Common Denomination. (ICD).
• Expiration date of those medicines. It must be less than 12 months minimum, and all medicines must fulfill the international quality standards.
• All medicines must be labeled and include instructions in one of the following languages: Spanish, English, or Portuguese. Medicines with instructions in any other language will not be accepted.
• Medicine labels must include the following information: generic name, potency, laboratory’s name, and expiration date if applicable.

b) Food:

• Expiration date and weight.
• Phytosanitary or Disinfection Certificate.
• Zoosanitary Certificate.
• Non-radioactivity Certificate.
• Export Certificate

c) Clothes, shoes, blankets, an d any other good by which diseases may be transmitted:

• Disinfection or Fumigation Certificate.

d) Medical Equipment and Materials:

• Brand, Model, weight, dimensions, age, and any other technical specifications required as the case may be.

e) Vehicles:

• Model, brand, weigh, age, technical specifications.
